If you would like to enrol with Kensington Health as a new patient, you will need to complete the new enrolment form and have available your identification (ideally a passport or a birth certificate).

Please review the Important Notice for all new patients and please read the New Patient Booklet before you start your enrolment with us.

You can enrol as a new patient with us by the following methods:

  • The quickest and easiest way to enrol with us is by completing the online form
Alternatively, you can also enrol by:
  • Call us – call our friendly reception team on (09) 437 9070 and they can get an enrolment form to you.
  • Come see us – call into our reception and they will give you a hard copy enrolment form.

When submitting your manually completed new patient enrolment forms:

  • Please ensure you complete all details requested on the new patient enrolment form and sign / date the form (note: we are unable to process incomplete forms)
  • Please ensure you bring in your passport, birth certificate or community services card (the only accepted forms of ID)

We encourage you choose to enrol with us via the online method – we will get you registered much faster than the other methods.

Important notice for all new patients

Please note: By submitting your new patient enrolment request, you automatically acknowledge and agree to the below terms and conditions and Kensington Health’s Terms of Trade / Credit Terms, as detailed in the New Patient Booklet. We encourage you to read this booklet before you start your new enrolment process.

Thank you for considering Kensington Health to be your trusted Your Whangārei Medical Centre & Whānau GP Service. If you decide to enrol with Kensington Health, you will be expected to attend a new patient assessment appointment. The purpose of this assessment is to introduce you to our practice and for us to get a good understanding of your medical history, what medications you are on, address any needs you have in a timely manner, and to understand you and your whānau in your health journey. 

Generally, one of our experienced nursing team will conduct this assessment. The all-inclusive fee for this assessment is $95.00 (including GST) – which will include any agreed prescription requests, and this fee will be payable at our reception desk when you check in for this new patient assessment appointment (i.e. the fee is payable prior to the assessment happening). 

During your assessment;

  • You may advise that you wish to see a doctor or nurse practitioner about a concern you may have
  • The nurse assessing you may suggest that they need to refer you to a doctor or a nurse practitioner for a specific reason (which they will explain to you in full)

If either of these needs should arise, a separate appointment will need to be made to see a doctor or nurse practitioner – and there will be a separate and standard appointment fee for this (for details of the fee applicable for these appointments, please refer to the New Patient Booklet, our reception area wall, our website – or just talk to our friendly reception team). 

Please note that once you are enrolled with us;

  • our standard appointments are for one or two issues maximum, depending on the complexity of those issues.
  • If you need to talk about more than two issues, either a separate standard appointment OR an extended appointment will need to be made, for which the relevant published appointment fee will be payable on the day of your appointment.
